 username_pm_notification.mod - Problem!

Hi, I have just installed the Name in PM mod - http://www.4webhelp.net/phpbb/username_pm_notification.mod

I have checked and checked again, but it does not show the Username of the Sender.

Is there something that has to be done to the Database or anything? Embarassed
